He’s Photographed Every Medical Receipt Since 2016. At 65, His HSA Owes Him $43,000 Tax-Free

Quick Read

  • The IRS sets no deadline for HSA reimbursements, letting savers accumulate decades of receipts and pull $43,000 or more tax-free at retirement.

  • Clark Howard confirms receipts from any year unlock a tax-free HSA withdrawal, as long as those expenses were never previously reimbursed or deducted.

  • Lose your receipts and the IRS reclassifies the withdrawal as ordinary income, adding a 20% penalty for anyone under 65.

  • Are you ahead, or behind on retirement? SmartAsset's free tool can match you with a financial advisor in minutes to help you answer that today. Each advisor has been carefully vetted, and must act in your best interests. Don't waste another minute; learn more here.

If you have a Health Savings Account, you already own the closest thing the tax code offers to a time machine. Pay a medical bill out of pocket today, photograph the receipt, and you can reimburse yourself, tax-free, five, ten, or thirty years from now. That is the HSA receipt-hoarding strategy, and it is why one 65-year-old with a decade of saved receipts is sitting on a $43,000 tax-free withdrawal he can pull whenever he wants.

The Buried Rule Inside Your HSA

Here is the quirk: the IRS never set a deadline for reimbursing yourself from an HSA. If you incurred a qualified medical expense after you opened the account, and you have not already claimed it on your taxes or been reimbursed by insurance, you can pay yourself back years or decades later. The HSA keeps growing, tax-free, until you do. Every receipt you keep is essentially a tax-free withdrawal token with no expiration date.

That is why the "photograph every receipt" crowd exists. They pay medical bills with a taxable checking account today so the HSA can compound untouched. When they want the money later, the paperwork unlocks it.

The Rule, In Writing

The authority is IRS Notice 2004-50, Q&A-39, which states an account beneficiary may defer reimbursement indefinitely as long as the expense was incurred after the HSA was established and was not otherwise reimbursed or deducted. The HSA itself is governed by Internal Revenue Code Section 223. Financial radio host Clark Howard put the mechanics plainly on his podcast: "Remember to keep your medical receipts all through the years, because if later in life you want to do a tax-free withdrawal from the HSA, it doesn't have to be withdrawn in the same year as the expense. You just have to have proof of the unreimbursed expenses you've had through the years."

Who Qualifies, Who Does Not

To contribute to an HSA in 2026 you must be enrolled in a qualifying high-deductible health plan, have no other disqualifying coverage, and not be enrolled in Medicare. The HDHP must carry a minimum annual deductible and capped out-of-pocket maximums set by the IRS each year. The 2026 HSA contribution limits are $4,300 for self-only coverage and $8,550 for family coverage, plus a $1,000 catch-up for account holders 55 and older. Once you enroll in Medicare, you can no longer contribute, but you can still reimburse yourself from your existing balance using old receipts. Spouses and dependents on your plan generate qualified expenses too.

How to Run the Strategy

  1. Max the HSA every year you are eligible and invest the balance rather than leaving it in cash. Even a conservative Treasury-based allocation is meaningful when the 10-year Treasury yield sits at 4.65% as of July 27, 2026, versus a national average 12-month CD rate of 1.68% on taxable cash.

  2. Pay medical bills, dental work, vision, prescriptions, and Medicare Part B and D premiums after 65 out of your regular checking account.

  3. Photograph every Explanation of Benefits and every receipt. Save them to a cloud folder labeled by year with the provider and amount.

  4. Log each expense in a spreadsheet: date incurred, provider, amount, whether insurance paid any portion.

  5. When you want tax-free cash, request an HSA distribution equal to your stockpile of unreimbursed expenses. Report it on Form 8889.

The Catch Nobody Mentions

Three traps sink this strategy. First, only expenses incurred after your HSA was established count. A bill from 2015 does not qualify if you opened the HSA in 2016. Second, you cannot double-dip. If you already deducted the expense on Schedule A or had a Flexible Spending Account cover it, it is off the table forever. Third, the IRS puts the documentation burden on you, not the custodian. Lose the receipts and lose the tax-free status: the withdrawal becomes ordinary income, plus a 20% penalty if you are under 65.

Inflation is also quietly eating your unreimbursed balance. CPI rose from 322.169 in July 2025 to 332.568 in June 2026, so a 2016 receipt buys less today than it did when you filed it. The tax-free withdrawal still works. The purchasing power does not wait.
